Registration is confirmed by receipt of at
least $100 deposit. Payment for registrations may be made in two ways:
online by PayPal (by clicking on the logo at the
bottom of the page). Please note that PayPal transfers must be made
from an account already containing the funds, or linked to a bank
account, rather than from a credit card.
by check or money order made out to Kammermusik, sent
to David Craig , Treasurer
of Kammermusik Workshops, 1120 N. Lake Shore Dr., Apt. 11C
Chicago, IL 60611. If you choose this method, please fill out, print
and enclose a copy of this registration form (but be sure to submit
it online as well).
Your reservation will be secured when the
payment is received. The balance of your charge must be paid no later
than the first day of the workshop itself. You may cancel your reservation
up to May 1, 2008 and will obtain a full refund of any payments. Cancellations
on or after May 1, 2008 will forfeit the $100 deposit (but any surplus
over $100 that you may have paid will be refunded).
The Board of Directors of Kammermusik reserves
the right to decline reservations at any time up to May 1 2008, if this
is necessary to preserve the musical balance of the workshop; in such
cases all monies paid will be refunded promptly in full. Applications
submitted after May 1, 2008 may be accepted at the discretion of the
President if space is available.
A private attendees web page will give details
of the program, the groups and what to bring with you. You will be given
a password for this page. It will be updated with this year's program
as it is developed.
We will need to know exact numbers for meals
for participants and accompanying persons by June 13 2008. If there
are any alterations to the form you submit here, please email the President .
Registrations will be limited to ten per instrument,
with a further criterion of a balanced set of groups. Priorities will
be determined by the order of submission of a registration form and
deposit.
