Registration Form

This form allows you to register for the Kammermusik Winter Woodwind Masterclass, Feb 18 - 20, 2012.

You should complete the ACMP self-rating guide before submitting your final reservation form, to assist in selecting players and in assigning parts. Please do this even if you are a regular participant in Kammermusik workshops, since we are using this guide for the first time this year.

You must complete fields marked with a * before submitting the form.

Kammermusik Workshops, Inc., does not handle lodging reservations. If you want lodging at Sunrise Springs, you should book your room directly with Sunrise Springs, reservations@sunrisesprings.com. Reservations with Sunrise Springs should be made by January 1, 2012 to secure the group discount rates.

You should receive acknowledgement of receipt of your registration within 3 days after its submission. If you have not, please contact the Workshop Director.

Participants will be selected at the entire discretion of Kammermusik Workshops, Inc., based on the need of the workshop.

After you have been notified of acceptance into the workshop, your registration will be complete and confirmed upon your full payment for the workshop. Payment for the workshop must be made no later than one week after notification that you have been accepted.

Payment for registrations may be made in two ways:

  1. online by PayPal (by clicking on the logo at the bottom of the page). Please note that PayPal is only possible if made from an account already containing the funds, or linked to a bank account, rather than from a credit card.
  2. by check or money order made out to Kammermusik, sent to David Craig, Treasurer of Kammermusik Workshops, 1120 N. Lake Shore Dr., Apt. 11C, Chicago, IL 60611. If you choose this method, please fill out, print and enclose a copy of this registration form.

You may cancel your reservation up to December 14, 2011 and obtain a full refund of any payments. If you cancel after this date, you will forfeit the full registration fee unless Kammermusik Workshops is able to fill your place.

We will need to know exact numbers for lunches by February 1, 2012. If you are bringing a guest for lunch and have not chosen this on your registration form, please email the Treasurer prior to that date and arrange payment for those additional charges.

After you have been accepted for the workshop and have paid your fee, you will be given an account on our private members' web page. Further information about the workshop will be posted on this page.
